If there are any documents such as reports, photos, or any other file you wish for Dr. Wetter to have and review in advance of your appointment, you will be able to upload the file as well.
Step 2: Please review and sign the "Office Policies and Consent for Treatment Form" and the "Notice of Privacy Form" by clicking on the screen buttons below (will open a new window and allow you to sign online, securely). Then, please select either the Adult, Adolescent, or Couples Intake Information Form to complete online (whichever best pertains to the focus of treatment). Please be sure to complete each document and submit before closing window; your information will not be saved if the window is closed before submitting. These documents will be received and reviewed by Dr. Wetter directly.
